Documents required for SL COE:
1） Copy of Application Form;
2） Three (3) passport size photos (3cm * 3cm) original + one (1) electronic copy (with the signature on the back of the photos )
3） Copy of Seafarer’s Passport;
4） Valid Medical Fitness Certificate (issued not earlier than 18 months)
5） Copy of Seaman Record Book or proof of marine experience;

6） Copy of Certificate of Competence (COC) and copy of Certificate of Watch Keeping (including seafarer’s name, date of expiration, capacity and limitations in English)
7） Copy of Basic Safety Training Certificates -- Personal survival techniques, Fire prevention and fire fighting, Elementary first aid, and Personal safety and social responsibilities.
8） Special training certificates for tankers, Ro-Ro/passenger vessels and passenger vessels.
Pls note:
1） For vessels sailing within A1+A2+A3 sea area, two (2) GMDSS certificates are required; for A1+A2 or A1 sea areas, one GMDSS certificates is required.
2） SSO is normally not required for vessels flying SL flag; however, it should be provided if required by particular port authority. Normally Vietnam ports have requirements.
3） Provisional certificates shall be issued directly after verification of the documents supplied. Valid period of provisional certificate is 3 months in general. However if the valid period of COC is less than 3 months, then this period shall apply.
4） Permanent certificates shall be issued normally two (2) months later.
