Following documents are required for Liberia crew licenses:
* Photos: size 3.5×3.5cm – 5cm×5cm, of color copy with resolution 150-300;
* Rating without watch-keeping requirement: Seafarer’s Passport, Professional Training certificate (page with photo); Basic Safety Training Certificate, Medical Fitness Certificate;

* Ratings with watch-keeping requirement: Seafarer’s Passport, Watchkeeping Certificate (photo page and English page); Professional Training Certificate (photo page); Basic Safety Training Certificate; Survival Craft and Rescue Boats Proficiency Certificate (for AB and Bosun), Medical Fitness Certificate;
* Officers: Seafarer’s Passport; Certificate of Competence (photo page and English page); Professional Training Certificate (photo page), Basic Safety Training Certificate; Medical Fitness Certificate and the certificate of Ship Security Officer if SSO endorsement is needed.
* Note: for oil/chemical tanker and LNG ships, related special training certificates (photo page) and other training certificates shall be required; all the certificate copies shall be smaller than 1MB and not necessarily in color.
